All parents deal with a child's bruises and bloody noses from time to time. For most kids, a bandage and a sympathetic hug is all it takes to get them back in the game. But a small number of kids might have constant nosebleeds, unexplained bruising, or prolonged bleeding after a tooth is pulled.

Excessive or prolonged bleeding could be a sign of a genetic disorder known as von Willebrand disease. For many people, the symptoms are so mild that they never even know they have the disorder. For those with a more severe form of the disease, a proper diagnosis and treatment plan can help them to live an active life.

About von Willebrand Disease
Von Willebrand disease, or vWD, is an inherited disorder that affects the blood's ability to clot properly. It takes its name from Dr. Erik von Willebrand, who first described the condition in 1926. As a group, bleeding disorders (including hemophilia) are rare. Von Willebrand disease is the most common inherited bleeding disorder, affecting as much as 1% of the population or more.

Normally, bleeding occurs when a blood vessel is cut or torn. Platelets (small cells that float in the blood) plug the hole in the blood vessel to stop the flow of blood. With the help of calcium, vitamin K, and a protein called fibrinogen, the platelets create a mesh to hold the plug in place and close the wound. As this mesh dries, it hardens and forms a scab to protect the wound as it heals.

A substance in the blood known as von Willebrand factor helps platelets stick to damaged blood vessels. Another function of von Willebrand factor is to carry the important clotting protein, called factor VIII, in the blood. People with vWD have a problem with one or both of these blood components.
Diagnosis and Treatment
Because the symptoms can be mild, vWD can be difficult to diagnose and often goes undetected.

Tests to diagnose vWD may include:

factor VIII level test (also called factor VIII coagulant), which measures the level of factor VIII and its ability to function
von Willebrand factor antigen test (also called factor VIII antigen), which measures the amount of von Willebrand factor and von Willebrand activity (ristocetin cofactor or RCF). The disorder is considered mild if a person has 20% to 49% of the normal amount. It is severe if the amount is 5% or less. Patients with type O blood may have reduced levels of von Willebrand factors.

platelet function tests, which determine how well the platelets work
Tests may need to be done more than once because these levels may rise and fall over time in an individual. Also, the doctor will take a family medical history to see if other relatives have a bleeding disorder.

The most common treatment for vWD is desmopressin, a medication that causes a temporary increase in the von Willebrand levels. It can be given intravenously (into a vein) or intranasally (into the nose), but may be ineffective in treating Type 2A or Type 2B.

Patients with Type 3 (and some with Type 2A and 2B) will need treatment with Humate P, a medication derived from human plasma that contains both factor VIII and von Willebrand factor that is given intravenously. Patients with Type 1 also might receive Humate-P in certain situations, such as major trauma or major surgery, although they can generally be treated with desmopressin.

Medication to inhibit the breakdown of blood clots also might be used, such as aminocaproic acid (Amicar).

Precautions
Kids with vWD should avoid unnecessary trauma, including contact sports. If bleeding does occur, apply pressure to the area. During nosebleeds, pinch the end of the nose and have the child lean slightly forward to keep the blood from flowing down the throat.

Because of the possibility of excessive bleeding, male infants who are at risk for vWD (those with a family history of the disease) should not be circumcised without a doctor's OK.

Girls with vWD who have started their periods might want to take extra pads or a change of clothes in case of accidents. Heavy menstrual bleeding can sometimes be controlled with birth control pills.

Call your doctor immediately if your child experiences any excessive or unexplained bleeding.

The 2 main treatment options for patients with vWD are DDAVP and transfusion therapy.

Desmopressin
Type 1 vWD
DDAVP is the treatment of choice for individuals with vWD type I. DDAVP is a synthetic analogue of the antidiuretic hormone vasopressin; it has enhanced antidiuretic activity and no pressor activity related to vasopressin. The infusion of DDAVP into healthy individuals and individuals with vWD type I results in a rapid increase in circulating levels of vWF:Ag and FVIII and RCoF activity.
Typically, a maximal rise of vWF and FVIII is observed in 30-60 minutes. The typical maximal rise is 2- to 4-fold for vWF and 3- to 6-fold for FVIII. Additionally, hemostatic levels of both factors are usually maintained for at least 6 hours. DDAVP can also be administered via a highly concentrated nasal spray; 300 mcg intranasally produces levels comparable to those observed with an intravenous infusion. Intranasal therapy is particularly useful for home therapy of menorrhagia and recurrent epistaxis.
Type III vWD
Individuals with vWD type III have a virtually complete deficiency of vWF; thus, the fact that DDAVP, an agent that causes release of stored vWF, has no effect in patients with vWD type III is not surprising.
The treatment of choice for patients with vWD type III (and other vWD types unresponsive to DDAVP) is virus-inactivated, vWF-containing FVIII concentrates that contain a near-normal complement of high molecular weight vWF multimers. Most experience reported in the literature has been with the use of Humate-P, a plasma-derived product of intermediate purity. Two other FVIII concentrates, Alphanate and Koate-HP, have been reported to be efficacious in the treatment of vWD. Too little vWF is present in monoclonally purified FVIII concentrates and recombinant FVIII concentrates to allow their use in the treatment of vWD.
Alloantibody formation occurs in 10-15% of patients with type III disease. Therefore, the possibility of this complication must be managed appropriately because patients are at increased risk for life-endangering anaphylactic reactions to vWF-FVIII preparations. With hemostatic stress in emergency situations, infusion of FVIII preparations devoid of vWF, while adjusting for the markedly decreased FVIII half-life, may be necessary.
Type II vWD
Responses to DDAVP are variable in patients with type II disease. A trial infusion may be performed to evaluate the potential efficacy for a particular patient.
Many individuals with vWD type IIA have a response to DDAVP, with peak vWF and FVIII levels at 30-60 minutes. This is similar to responses observed in patients with vWD type I; however, rapid loss of vWF, FVIII, and, particularly, RCoF activity, occurs as the high molecular weight multimers are degraded, with return to baseline levels at 4 hours postinfusion. Although the response is transient, it may be adequate therapy in certain clinical situations.
For prophylaxis in major surgery or for treatment of serious bleeding episodes, vWF-containing FVIII concentrates are the treatment of choice. Franchini et al performed a literature review of when it is necessary for antithrombotic prophylaxis with vWF/FVIII concentrates in patients with vWD who undergo major surgery.[11] The investigators noted that only 11 cases have been reported in which venous thromboembolic complications occurred in surgical vWD patients who were prophylactically treated with such concentrates, most during orthopedic procedures. Franchini et al advised the need for thromboprophylaxis in such patients should be individually assessed, after a careful risk/benefit analysis.[11]
DDAVP trials may be contraindicated in patients with type IIB because of thrombocytopenia and possible thrombotic complications.
DDAVP is probably not effective in patients with type IIM and is rarely effective in patients with type IIN.
Platelet transfusions
These may be helpful in some patients with vWD whose disease is refractory to other therapies.
Cryoprecipitate and fresh frozen plasma contain functional vWF but should be avoided whenever possible because of the potential transmission of viral disease. An additional drawback of fresh frozen plasma is the large infusion volume required.

If such a big movement can't make the bill pass, it will never ever pass and India will never become a corruption free country. Come on friends dont let the movement stop.

He is advocating for a Bill, The Jan Lokpal Bill (The Citizen Ombudsman Bill), that will form an autonomous authority who will make politicians (ministers), beurocrats (IAS/IPS) accountable for their deeds.

7. It's an entirely new thing right..?

In 1972, the bill was proposed by then Law minister Mr. Shanti Bhushan. Since then it has been neglected by the politicians and some are trying to change the bill to suit thier theft (corruption).

8. Oh.. He is going on a hunger strike for that whole thing of passing a Bill !

How can that be possible in such a short span of time? The first thing he is asking for is: the government should come forward and announce that the bill is going to be passed. Next, they make a joint committee to DRAFT the JAN LOKPAL BILL. 50% goverment participation and 50% public participation. Because you can't trust the government entirely for making such a bill which does not suit them.

9. Will his fast have any impact on the deaf Government of India?

Yes, it certainly would. Because, last time when Anna sat on fast -

* 6 corrupt ministers in Maharashtra had to resign

* 400 corrupt officers were dismissed from job

* 2002 - Maharashtra RTI Act was passed

* 2006 - Central Government withdrew its proposal to amend Central RTI Act

10. Fine, What will happen when this bill is passed?

A LokPal will be appointed at the centre. He will have an autonomous charge, say like the Election Commission of India. In each and every state, Lokayukta will be appointed. The job is to bring all alleged party to trial in case of corruptions within 1 year. Within 2 years, the guilty will be punished. Not like, Bofors scam or Bhopal Gas Tragedy case, that has been going for last 25 years without any result.

11. Is he alone?

Whoelse is there in the fight with Anna Hazare? Baba Ramdev, Ex. IPS Kiran Bedi, Social Activist Swami Agnivesh, RTI activist Arvind Kejriwal and many more. Prominent personalities like Aamir Khan, Anupam Khair, etc. are supporting his cause.

12. Ok, got it. What can I do?

At least we can spread the message. How? Putting status message, links, video, changing profile pics. At least we can support Anna Hazare and the cause for uprooting corruption from India. At least we can hope that his Hunger Strike does not go in vain. At least we can pray for his good health.

Salient features of Jan Lokpal Bill
Drafted by Justice Santosh Hegde, Prashant Bhushan and Arvind Kejriwal, this Bill has been refined on the basis of feedback received from public on website and after series of public consultations. It has also been vetted by and is supported by Shanti Bhushan, J M Lyngdoh, Kiran Bedi, Anna Hazare etc. It was sent to the PM and all CMs on 1st December.

An institution called LOKPAL at the centre and LOKAYUKTA in each state will be set up

Like Supreme Court and Election Commission, they will be completely independent of the governments. No minister or bureaucrat will be able to influence their investigations.

Cases against corrupt people will not linger on for years anymore: Investigations in any case will have to be completed in one year. Trial should be completed in next one year so that the corrupt politician, officer or judge is sent to jail within two years.
4.
The loss that a corrupt person caused to the government will be recovered at the time of conviction.
5.
How will it help a common citizen: If any work of any citizen is not done in prescribed time in any government office, Lokpal will impose financial penalty on guilty officers, which will be given as compensation to the complainant.
6.
So, you could approach Lokpal if your ration card or passport or voter card is not being made or if police is not registering your case or any other work is not being done in prescribed time. Lokpal will have to get it done in a month's time. You could also report any case of corruption to Lokpal like ration being siphoned off, poor quality roads been constructed or panchayat funds being siphoned off. Lokpal will have to complete its investigations in a year, trial will be over in next one year and the guilty will go to jail within two years.
7.
But won't the government appoint corrupt and weak people as Lokpal members? That won't be possible because its members will be selected by judges, citizens and constitutional authorities and not by politicians, through a completely transparent and participatory process.

What if some officer in Lokpal becomes corrupt? The entire functioning of Lokpal/ Lokayukta will be completely transparent. Any complaint against any officer of Lokpal shall be investigated and the officer dismissed within two months.
9.
What will happen to existing anti-corruption agencies? CVC, departmental vigilance and anti-corruption branch of CBI will be merged into Lokpal. Lokpal will have complete powers and machinery to independently investigate and prosecute any officer, judge or politician.
